Rates & Freight Quote ** All shipments are Prepaid with a $125 min. charge (this does not include fuel surcharge). Please note: This quote will be based on the information you provide. Actual charges will be determined by shipment characteristics and any additional service options applicable at time of shipment. Shipping Terms & DescriptionsCOD Fee: FSD, at the request of the consignor, will collect a COD amount from the consignee and an additional fee will be applied. Hazardous Materials (Haz Mat):Hazardous commodities are those defined in the code of Federal Regulation, Title 49, by the US Department of Transportation. FSD will accept shipments of hazardous commodities subject to the terms of tariff 100 item 540. A hazardous commodities handling fee will be assessed for any shipment containing hazardous commodities. Inside Delivery:When FSD is requested to deliver freight to a position beyond the immediate adjacent unloading position, an additional charge will apply. Inside Pickup:When FSD is requested to pick up freight from a position beyond the immediate adjacent loading position, an additional charge will apply. Liftgate at Origin/Destination:When requested by the customer, FSD will provide lifting or lowering devices to accomplish pickup for additional fee. Limited Access Locations (Pickup or Delivery Service):This includes but is not limited to the following: Individual (mini) storage units, Construction sites, Fairs or carnivals, Prisons, Military Base/Installations, or Mine Sites. In the case of delivery, such charge will include an initial notification to make delivery arrangements only if Carrier is provided the correct telephone number of the Consignee. Notify Before Delivery:FSD will provide notification prior to delivery when requested for an additional fee. Perishable:Protection From Freezing Service will only be provided when: Suitable equipment is available, The outside temperature is above10 degrees Fahrenheit for shipments moving in all standard lanes, Consignor endorses the bill of lading "protect from freezing", or words of similar purport, Such markings will be on each package when shipment weighs less than 6,000 lbs., or The commodities being shipped have a freezing point of 32 degrees or less. ProActive Notification:ProActive Notification only applies to Time Critical, Time Advantage or Guaranteed Day/MultiDay Shipments Residential Pickup/Delivery:FSD will assess a charge when required or requested to make a delivery at a non-Commercial location. The term non-Commercial applies to private residences, apartments, churches, schools, camps, and other such locations not generally recognized as commercial business locations. Single Shipment:A single shipment of less than 500 pounds, picked up at one time and place, will be subject to an additional charge. Tradeshow Delivery:A charge will be accessed when delivered to a trade show location. This also applies to deliveries made to the advance warehouse, where shipments are held prior to the tradeshow. Tradeshow Pick-up:A charge will be accessed when picked up at a trade show location. This also applies on show to show shipments.
